Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. The wound vacuum used to heal her transmetatarsal amputation (TMA) was turned off by a CNA on 1/19/25 from 7AM - 3:30PM and a family member had to turn it back on during his visit. 2. On one evening, she did not receive her insulin shot and her blood sugar was not taken. 3. She did not receive a food tray for dinner on 1/20/25 and when she asked the CNA she was told that they did not have a food tray for her for dinner. Her meals are essential for helping her receive enough protein and vitamins to help heal her wound. 4. On 1/16/25 upon arriving at the facility, the bathroom had feces on and around the toilet and was unsanitary. A family member had to clean the toilet. Then on Saturday, 1/18/25 her bathroom toilet was clogged and unsanitary to use. Upon calling the facility, a kind staff member came to my mother's room to ensure her that the bathroom would be unclogged that same night or early the next morning.
