Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

My mother was a long term resident at Boulevard for about two and a half years. After two hip replacement, two strokes, many falls and other medical issues, home health care was just not enough anymore. As much as my mother did not want to leave her home, my brother and I had no choice but to seek more care for my mother. She lived up the road from Boulevard and we knew of the place as her husband was a resident there fifteen years prior. My mother was not an easy resident as her mind was sharp, but her health issues continued to take a major toll on her body and mobility. I have seen first hand the compassion and help that the staff provided to and for my mother. Each time I would visit my mother at all times of the week, the place was extremely clean and welcoming. My mothers room was always well kept and many times I would see the staff helping and caring for my mother. Paulette was the Unit Manager on the wing that my mother was staying and she always would provide my brother and I with updates and the status of my mothers condition. She returned phone calls and I have to add spoke with my brother and I with suggestions to help aide us with my mothers conditions. I met many CNA's that cared for my mother and all knew her by name and I always seen them care for my mother with compassion. I had the opportunity to talk with the Administrator named Tim and the Director of Nursing, Tanisha. Every time I would visit they were always out and about and interacting with not only the families, but the residents as well. I have seen them stop and talk with residents like they were life long friends. They new their names and situations and it always made me smile to see the care and compassion they have provided to all. They truly care and it shows. It shows with most of the staff I saw every time I would visit. It is truly refreshing these days to witness a collective group of people care for our loved ones as we would. My mother passed away two weeks ago at the age of 84. I wanted to take a moment to stop and say thank you with my heart to all the staff at Boulevard. To watch a loved relative health fail and life slipping away is one of the hardest things a person has to do in life no matter what your faith my be. Boulevard helped me get through these past couple of difficult years. Thank you Boulevard for your caring a compassion and for providing such care. He couldn't get out of bed and needed to use bathroom. No portable container given. No one showed for 45 minutes. Needless to say he made a mess of bed and CNA was upset. Really. CNA tried to move him from chair and he fell onto bed hurting hip and back. Signed papers to get him out and they wouldn't give us copies of paperwork. Wouldn't give to me to take a picture. I was discharged on October 15th this year (2024). This place leaves much to be desired. If you need to come here or bring a loved one here just be sure they have an advocate. I pretty much was able to advocate for myself. I was there for 5 days before my medication was given to me, so you must be patient and settle in before you actually receive the care you came there for. One amazing nurse that is really on the ball is Bastia. She resolved so many issues. Marie is really a sweetheart. She is so easy going. I can’t say the same for Paulette. When you are lied to, it’s hard to rebuild trust. I was taking 3 bags of antibiotics daily. It didn’t start until I was there for five days. Bastia straightened that out, as she did other issues as well. I had arrived there from Delray Medical, I had open heart surgery in July. The first rehab after surgery was was The Legacy in Boca Raton. That place was the most God awful nightmare. When the people at Boulevard complained about it. I said, “If you think this place is bad here go to the Legacy.” Then you will love Boulevard. The stop date for my antibiotics was Oct. 13th and I was uncertain if it had been taken into consideration that my treatment didn’t start for the first five days upon my arrival. So when Bastia came in my room. To give it to me she said it was inclusive (meaning the three bags per day for the 13th.) I understood and Bastia went home and I dealt with Paulette. I was told that I was finished with the antibiotics. Paulette became adamant and said to me, “No you are finished there are no more bags of antibiotics left” I said, “Then why did Bastia tell me there were 7 bags left.” The nurse (Mike who I just love) said I can give you the your last few bags today if you want. I said, “ I will be fine I’ll talk to my infectious disease doctor, who said, I was to take all that were sent to me! He ordered the an RX oral ones for me to take at home. You have a gal there that’s a CNA I think named Sophia, and that woman will stop at nothing to be sure you have everything you need. The laundry situation needs a total so over. They ended up loosing a bag of my clothes. I purchased something from Walmart it was Imodium ad and the took it and never gave it back. You can see how many wonderful things there are about this place. After a bit of discord with Paulette, I was told I would be discharged on Oct.15th and that was fine but I never understood why it took 6 people to charge into my room to tell me. All I needed was the charming little pigtailed girl that asked me to sign the paperwork. She’s the sweetest . They are a bit understaffed like anywhere else so sometimes things take a little long but all in all, I gotta be here another four weeks so I just need to take it and roll with the punches. I have Marie in the morning at seven in the morning and she’s a lifesaver. I have 11 o’clock shift the worst she doesn’t want to work. Even after speaking to a supervisor and she said she’ll tell her she still does nothing. She does the minimum which is really not good but like I say there were other ones who are very, very good. Glenora in central staffing is excellent. When she made a reservation for me with AMR Ambulance to take me to my appointment she did everything perfect! A good thing I checked on AMR because my appointment is at 1 o’clock and they want to pick me up at 1 o’clock not good so I suggest if anybody comes in, you must follow up on your own. You know the saying is when you want something done do it yourself? But after I talked to Glenora, she is also calling them to make sure this is all taken care of the right way. Like I said before she is excellent, and you can rely on her. Also , they lost some of my clothes I have hardly nothing left. Luckily Dawn, who is very nice and is really working hard on finding my stuff said she said she should find it today. I’ve been here a week so let’s pray that this happens. This is not unusual in a nursing home, so I suggest if you ever come here have them put a note in the closet that somebody in your family is going to do the laundry because otherwise it takes forever or gets lost. That part is not good. But I must tell you I’ve being in a lot of places and this one is decent snd very clean and they do listen to you when things are wrong. Most of the staff is very good and they do listen to what you say. But if You expect to get attention right away don’t even bother coming into one of these and get a private nurse at home which if I could afford I would’ve done. You hope that they will get the best level of care. I placed my grandfather here due to his late-stage dementia. He arrived at the beginning of September being able to walk (with his walker), talk, dress and feed himself. By the first week of October my grandfather was severely dehydrated, could no longer talk, couldn't dress or feed himself, and he had a fall in the facility. His rapid decline was very alarming and hard to witness. My husband and I took him to his neurologist who told us he needs to be hospitalized immediately. After addressing our concerns with the head of nursing about his level of care and how he needs to be placed in the ER, they stated that they treat their residents like "family" and there is no need to hospitalize him because they were doing the same tests the hospital would do. 2 days later my grandfather had to be sent to the ER because he was lethargic, not eating, and couldn't swallow. Within a week of his hospital stay he went into cardiac arrest and passed away within 3 days. Not ONCE did I receive a call from this facility to check in on my grandfather or to send their condolences after he passed away. When I arrived to pick up his belongings that were already sent to storage, I did not receive any feedback from head of staff. To say your treat your residents like "family" is completely hypocritical and it showed through your lack of care. My grandfather was neglected, period. I will say that there were a couple of very caring nurses, but overall, I'm extremely disheartened and unsatisfied with my grandfather's care at this facility. I loved him dearly and now he is no longer with us. I brought it to the attention of the nurse around 8 am. (3rd day), and she relayed it to whoever is above her. About two hours later, I got a phone call from some administrator asking what the problem was. Mean while this woman was in the building talking to me on the phone. She told me she would have to be discharged the following day due to the doctors order (the doctor that didn't even see her). At this point, it was my mother's second night/third day being in this horrible place. I told the administrator, "Do what you have to in order to get her discharged. She needs to leave today." Now I get a second phone call with more than one administrator on the phone. They tell me they are in a meeting, and they want to resolve what's happening with my mother's care. I then tell them it's ridiculous to talk to me on the phone when you can come to the room. About 20 mins later, there are 4 administrators in the room talking to me and my mother, and they tell us she can't be discharged. So, of course, my mother says I'm leaving regardless. As any good son would do, I told them that if they don't discharge her in the next 30 mins, we're leaving on our own. Which is what finally happened. If you care about your loved ones, DO NOT BRING THEM TO THIS HORRIBLE FACILITY. out of this whole experience, there was one nurse who was great, I can't recall her name, but she works the night shift, and if I can remember correctly, she's of Eastern European descent. I really hope she works somewhere else now. The only reason this place is in business and can get away with the treatment they provide is because it's all paid for by Medicare and / or Medicaid. We went to them all and toured each one. We went to Bouelvard last, we should have gone there first. First of all during the tour the staff at Boulevard were extremly friendly, compassionate and caring they took the time to explain the therapy and treatmetns that grandmom would receive. They answered many questions and even encouraged us to talk with other families and patietns. TWe did. We met a number of patinent and their families; who were visiting, we talked with about six families, they all were extremely hjappy. Three of them told us that they had actually transfered from two other nursing centers in town that we had also looked at. One had spent a million or so dollars on rennovations,had an internet cafe... but if your staff are miserable and unhappy, and the patients were all complaining aout the staff speaking in creole in the hall ways, and unaswered call lights..we witnesed an aide go in and shut off a light and leave... well....The other told us about their expereince at another large facitliy..the buildign was newer,,but bricks and glass and brass do not proivde care, and apprently neither did their staff.. So we went to Boulevard, and we have not been disappinted. The staff there get it. They are focused on the patients,, they all seem to get along and truly love their jobs. I found out that many of them have or had family members in there. I can understand why. The Center is extremly well run, it is clean, and cheerful. My mom does not worry about her mom being at Bouelvard, I could tell you that we would have in the other nursing centers. I want to say one more thing..if you are out of town like up north and your family member is here...do not go anyplace else, you can trust the staff at Boulevard to care for your loved one like they are their own family. The staff make all the difference in the world, the nurses aides were all caring and compassionate. The nurses wsere very pleawseant and competent. The therapy staff exceptional.
