Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

[review] => I want to start by thanking all the staff at Boulevard for the care and compassion each provided to my mother.
My mother was a long term resident at Boulevard for about two and a half years. After two hip replacement, two strokes, many falls and other medical issues, home health care was just not enough anymore. As much as my mother did not want to leave her home, my brother and I had no choice but to seek more care for my mother. She lived up the road from Boulevard and we knew of the place as her husband was a resident there fifteen years prior.
My mother was not an easy resident as her mind was sharp, but her health issues continued to take a major toll on her body and mobility. I have seen first hand the compassion and help that the staff provided to and for my mother. Each time I would visit my mother at all times of the week, the place was extremely clean and welcoming. My mothers room was always well kept and many times I would see the staff helping and caring for my mother. Paulette was the Unit Manager on the wing that my mother was staying and she always would provide my brother and I with updates and the status of my mothers condition. She returned phone calls and I have to add spoke with my brother and I with suggestions to help aide us with my mothers conditions.
I met many CNA's that cared for my mother and all knew her by name and I always seen them care for my mother with compassion. I had the opportunity to talk with the Administrator named Tim and the Director of Nursing, Tanisha. Every time I would visit they were always out and about and interacting with not only the families, but the residents as well. I have seen them stop and talk with residents like they were life long friends. They new their names and situations and it always made me smile to see the care and compassion they have provided to all. They truly care and it shows. It shows with most of the staff I saw every time I would visit. It is truly refreshing these days to witness a collective group of people care for our loved ones as we would.
My mother passed away two weeks ago at the age of 84. I wanted to take a moment to stop and say thank you with my heart to all the staff at Boulevard. To watch a loved relative health fail and life slipping away is one of the hardest things a person has to do in life no matter what your faith my be. Boulevard helped me get through these past couple of difficult years. Thank you Boulevard for your caring a compassion and for providing such care.

At Boulevard I have been blessed to meet Allison and Sophie the activities director. For me it would be hard to be pleasant to everyone, but they do a great job. [review] => Boulevard Rehab in Boynton beach was the most horrific facility I've come across. Husband had hip replacement done and was sent there for inpt rehab. Was put in a bed and laid there for 2 hrs before anyone came to see him. No intercoms. Press a button and light outside door is suppose to grab someone's attn. He couldn't get out of bed and needed to use bathroom. No portable container given. No one showed for 45 minutes. Needless to say he made a mess of bed and CNA was upset. Really. I had to clean and change him. Forget about getting any meds. No hospital band. No scanning anything to verify drugs given. Had to take him out after 2 days. CNA tried to move him from chair and he fell onto bed hurting hip and back. Signed papers to get him out and they wouldn't give us copies of paperwork. Wouldn't give to me to take a picture. Name tags all on backwards. Ask their name several times and they mumble or can't understand them. Will be reporting them everywhere I can.

I being the *** and care taker for my mother for 20+ years understand that there are many stressors in healthcare when it comes to the lack of resources but my mother being treated at boulevard rehab was unacceptable. I had to call 911 myself to get my mother out of there. Let me add that my mother was admitted into the hospital once again! If you care about your love one do not! I mean do not! send them there unless you want them to die. I want to add that the administration team and staff did not care about any of concerns since day one. The nurses aides are not knowledgeable about dialysis patients. It seems as if every patient gets the same diet. My mother came out looking like Oompa Loompa because they just kept giving her fluids and food she was not supposed to be eating. This whole process has been mentally traumatizing for me. When I asked the Director of nursing (******* ********)if we can speak on the last day my mother was there she completely refused and stated she had a meeting to attend while she was discussing my mothers case with the social worker in front of my mother room. [review] => I came here to Boulevard sometime in September, I was here about a month. I was discharged on October 15th this year (2024). This place leaves much to be desired. If you need to come here or bring a loved one here just be sure they have an advocate. I pretty much was able to advocate for myself. I was there for 5 days before my medication was given to me, so you must be patient and settle in before you actually receive the care you came there for. One amazing nurse that is really on the ball is Bastia. She resolved so many issues. Marie is really a sweetheart. She is so easy going.
I can’t say the same for Paulette. When you are lied to, it’s hard to rebuild trust. I was taking 3 bags of antibiotics daily. It didn’t start until I was there for five days. Bastia straightened that out, as she did other issues as well. I had arrived there from Delray Medical, I had open heart surgery in July. The first rehab after surgery was was The Legacy in Boca Raton. That place was the most God awful nightmare. When the people at Boulevard complained about it. I said, “If you think this place is bad here go to the Legacy.” Then you will love Boulevard.
The stop date for my antibiotics was Oct. 13th and I was uncertain if it had been taken into consideration that my treatment didn’t start for the first five days upon my arrival. So when Bastia came in my room. To give it to me she said it was inclusive (meaning the three bags per day for the 13th.) I understood and Bastia went home and I dealt with Paulette. I was told that I was finished with the antibiotics. Paulette became adamant and said to me, “No you are finished there are no more bags of antibiotics left” I said, “Then why did Bastia tell me there were 7 bags left.” The nurse (Mike who I just love) said I can give you the your last few bags today if you want. I said, “ I will be fine I’ll talk to my infectious disease doctor, who said, I was to take all that were sent to me! He ordered the an RX oral ones for me to take at home. You have a gal there that’s a CNA I think named Sophia, and that woman will stop at nothing to be sure you have everything you need. The laundry situation needs a total so over. They ended up loosing a bag of my clothes. I purchased something from Walmart it was Imodium ad and the took it and never gave it back. You can see how many wonderful things there are about this place. After a bit of discord with Paulette, I was told I would be discharged on Oct.15th and that was fine but I never understood why it took 6 people to charge into my room to tell me. All I needed was the charming little pigtailed girl that asked me to sign the paperwork. She’s the sweetest .

[review] => I’ve been to many facilities (4) and the one in Manhattan was the best which Manhattan is always the best. I’ve been one in Staten Island and that was the worst. I’ve been to Long Island and that was horrible so believe it or not this place is half decent. But unfortunately, there are people that don’t want to work and other people that do an excellent job. . They are a bit understaffed like anywhere else so sometimes things take a little long but all in all, I gotta be here another four weeks so I just need to take it and roll with the punches. I have Marie in the morning at seven in the morning and she’s a lifesaver. I have 11 o’clock shift the worst she doesn’t want to work. Even after speaking to a supervisor and she said she’ll tell her she still does nothing. She does the minimum which is really not good but like I say there were other ones who are very, very good. Glenora in central staffing is excellent. When she made a reservation for me with AMR Ambulance to take me to my appointment she did everything perfect! A good thing I checked on AMR because my appointment is at 1 o’clock and they want to pick me up at 1 o’clock not good so I suggest if anybody comes in, you must follow up on your own. You know the saying is when you want something done do it yourself? But after I talked to Glenora, she is also calling them to make sure this is all taken care of the right way. Like I said before she is excellent, and you can rely on her. Also , they lost some of my clothes I have hardly nothing left. Luckily Dawn, who is very nice and is really working hard on finding my stuff said she said she should find it today. I’ve been here a week so let’s pray that this happens. This is not unusual in a nursing home, so I suggest if you ever come here have them put a note in the closet that somebody in your family is going to do the laundry because otherwise it takes forever or gets lost. That part is not good. But I must tell you I’ve being in a lot of places and this one is decent snd very clean and they do listen to you when things are wrong. Most of the staff is very good and they do listen to what you say. But if You expect to get attention right away don’t even bother coming into one of these and get a private nurse at home which if I could afford I would’ve done. I’ve waited 45 minutes up to 1 1/2 hours. All the therapists are excellence and that’s what’s very important, which is the most important!

Then once they were told I watched them sit there before going to check on him. Then waiting for them to bring me a towel and washcloth to wipe my love one off after they had an accident. It took them over 30 minutes and me speaking to four nurses. After they tried to explain why they were taking so long, but their excuse was sorry because I was standing there when they were notified about the gentleman. Then they did not properly clean the urine off the floor. Yelp it’s time to move my mom!

[review] => My partner James Tippett was in Boulevard rehab for 7 weeks after a stay in Bethesda hospital. He has many health issues as well as walking difficulty. I cant say enough about the care and compassion from the nurses and CNAs. They went above and beyond to help him throughout his stay as well as keeping me calm and sane. They are all kind, sweet and wanted the best for him. The physical therapy was also wonderful. They made him work hard when he wanted to give up. The food was very good and when he wanted extra portions they were there for him. The administrative team always answered my questions with kindness. I can honestly say after spending so much time there im truly going to miss our nurses and aids. Im not going to call out any names but those wonderful guys and gals know who you are. [review] => Placing your loved one in a long term care facility is one of the hardest decisions you'll ever make. You hope that they will get the best level of care. I placed my grandfather here due to his late-stage dementia. He arrived at the beginning of September being able to walk (with his walker), talk, dress and feed himself. By the first week of October my grandfather was severely dehydrated, could no longer talk, couldn't dress or feed himself, and he had a fall in the facility. His rapid decline was very alarming and hard to witness. My husband and I took him to his neurologist who told us he needs to be hospitalized immediately. After addressing our concerns with the head of nursing about his level of care and how he needs to be placed in the ER, they stated that they treat their residents like "family" and there is no need to hospitalize him because they were doing the same tests the hospital would do. 2 days later my grandfather had to be sent to the ER because he was lethargic, not eating, and couldn't swallow. Within a week of his hospital stay he went into cardiac arrest and passed away within 3 days. Not ONCE did I receive a call from this facility to check in on my grandfather or to send their condolences after he passed away. When I arrived to pick up his belongings that were already sent to storage, I did not receive any feedback from head of staff. To say your treat your residents like "family" is completely hypocritical and it showed through your lack of care. My grandfather was neglected, period. I will say that there were a couple of very caring nurses, but overall, I'm extremely disheartened and unsatisfied with my grandfather's care at this facility. I loved him dearly and now he is no longer with us.

All other personnel were nice, EXCEPT the main nurses station in the south wing. They are awful. If you go up to ask a question, you stand there just waiting until they finish their personal conversations and without even saying "I'll be with you in a minute." They don't even answer the telephone when you call from outside. It happened numerous times when I was there, but also months ago when my cousin was there and his daughter tried to call in with questions. We don't bother the desk unless it's very important so it's not like we're complaining for nothing.

I am proud to say that Boulevard has improved my Physical situation concerning my Health issues. I was bed bound flat on my back for 10 Months unable to move and was also placed on Hospice. In February of 2024 I Heard music in the Activity Department that encouraged me to get up and i watched the live Entertainment which I really enjoyed. After much encouragement from staff and my roommate i am now doing physical therapy 3 times week, I am completely off hospice, I'm in a brand-new wheelchair moving about freely. I'm able to transfer from wheelchair to bed on my own. I want to thank my Therapists Ingrid and Elien for working with me daily. And last but by no means least the amazing staff in Activity department who keeps me entertained and endure that my Physo Social needs are met on a daily basic. Boulevard is the place to be Ilive here and I am Happy and Comfotable.

[review] => I'm not sure what happened to my review that I left back at the end of January 2024. But I brought my mother here, which was supposed to be for 3 or 4 days. The experience was horrible. She was never seen by an actual doctor at all. After the second night, she didn't even want to stay there anymore. So that being said, I asked for her to be discharged so I could take care of her in the comfort of her own home. I brought it to the attention of the nurse around 8 am. (3rd day), and she relayed it to whoever is above her. About two hours later, I got a phone call from some administrator asking what the problem was. Mean while this woman was in the building talking to me on the phone. She told me she would have to be discharged the following day due to the doctors order (the doctor that didn't even see her). At this point, it was my mother's second night/third day being in this horrible place. I told the administrator, "Do what you have to in order to get her discharged. She needs to leave today." Now I get a second phone call with more than one administrator on the phone. They tell me they are in a meeting, and they want to resolve what's happening with my mother's care. I then tell them it's ridiculous to talk to me on the phone when you can come to the room. About 20 mins later, there are 4 administrators in the room talking to me and my mother, and they tell us she can't be discharged. So, of course, my mother says I'm leaving regardless. As any good son would do, I told them that if they don't discharge her in the next 30 mins, we're leaving on our own. Which is what finally happened. If you care about your loved ones, DO NOT BRING THEM TO THIS HORRIBLE FACILITY. out of this whole experience, there was one nurse who was great, I can't recall her name, but she works the night shift, and if I can remember correctly, she's of Eastern European descent. I really hope she works somewhere else now. The only reason this place is in business and can get away with the treatment they provide is because it's all paid for by Medicare and / or Medicaid. I honestly don't even see how they can operate the way they do. EXTREMELY INHUMANE. STAY FAR AWAY, FAIR WARNING PEOPLE

[review] => My sister was at Boulevard Rehabilitation Center for three weeks for post-surgery rehab. From admissions to discharge I was impressed by the quality of care she received. My first impression was the cleanliness of the facility. The floors shined and there wasn't the typical nursing home smell. My sister's room was spotless and was kept clean by a conscientious housekeeping staff. In addition, the quality of the food was excellent. The physical and occupational therapists were top-notch as well as the entire patient care team. I researched all of the rehab centers that were in-network with her insurance and chose Boulevard because of their high rating . Anyone reading this should not hesitate to place their loved-one there. I wish I could give them six stars!

The aides are unreal and will argue with you when you ask them why something is or why it smells so bad in the room. If they need feeding, good luck. During this time, the company has not returned most phone calls, barely answers the phone, voicemail boxes are full or not set up - communication, in general, has been almost non-existent.We found a facility and had her approved to move in on 12/11 and Boulevard has had excuse after excuse about why they are unable to discharge her. First excuse was they needed 72 hours to do paperwork even though the new facility said they had all the paperwork they needed. Next excuse was that they were worried about moving my stepmother because of her dementia. I told them that due to communication issues, we wanted to move her anyways.Once they received my approval, I asked that they move her the same day and they said they would reach out to the new facility and try to make arrangements but they never did. Now, they are not answering my calls, no matter what number I call to or from.

[review] => The hospital sent my friend to Boulevard Rehabilitation Center. It was terrible. He had a feeding tube in him, and I saw nurses dropped the feeding tube on the floor, picked it up, and stuck it back into his stomach. I had also seen the oxygen that went into his nose fall on the floor. They picked it up, and put it back in him. They also lied to me.
I live in Florida. This was in December, so it was a very cold day. I went into the room, and the room was freezing cold. My friend was shivering and shaking. I said to the nurse, 'Something's wrong because it's freezing cold in this room.' They moved him to the other side of the room, where there was an air conditioner like something you would see in a motel. Then another time I went in there, and the room was stifling hot. It was because they pushed the bed up against the air conditioner because he was a fall risk. They didn't want him to get out of the bed. My friend was there for 15 days, and he was on pureed food. The facility was clean though.

He has a feeding tube which I seen them pick up after they say he pulled it it was on the floor they just picked it up and put it back in from the dirty floor also he is on portable oxygen which the tube that goes into his nostrils was laying on the floor same scenario picked it up off the dirty floor and stuck it back in his nose. If you look at the board to see what nurses have which rooms you will see that most that most nurses have 8 rooms with 2 people in a room that is 16 patients. Instead of getting better he is getting worse.confusion disoriented anxiety.Humana is his insurance and they are wasting his time and benefits by suggesting this place.he has been there since December 18th he goes to dialysis 3x a week by palm Tran he is gone from 1-7 so when do they do his physical occupational and speech therapy on those days.The place is shameful.I am a poa for him and Humana will be getting a phone call.i will be taking pictures of the filthy sheets and room conditions Be smart do not send your loved one here

[review] => Unbelievable! So glad Uncle's stay was only 2 weeks or he may not have survived in this facility. Something as simple as getting a drink of water and hydration was impossible. I arrived at 11am and Uncle's mouth was so dry he couldn't speak (he was completely dehydrated). No water in the hallway dispenser... completely empty. I asked several workers, several times to please get him some more water and get the dispenser refilled. They ignored me. Didn't happen. I went to their kitchen to get Uncle's drinking cups filled. I even spoke to the Director personally about it. When I was leaving at 3pm, after 4 hours, the hallway dispenser was still empty! I picked up the dispenser and carried it to their kitchen to get it refilled with water. No help, no one cared. Unreal! This was just one example I witnessed of absolute disregard for the well-being of their residents. I am a CNA and have worked in Elder care for over 10 years. It breaks my heart to see this lack of respect and lack of care for this amazing generation. Considered contacting the Ombudsman.

The staff and I got along very well, my Aunt was very comfortable leaving. My Uncle enjoyed talking to the staff about his army stories, even thought it was repetitive no one seemed to mind. It seems to me everyone was easily approachable if we needed anything. The rooms are kept clean and bright. The same thing that a lot of people are upset about on here is what happened to us. The exact same thing. The staff and nurses are friendly until you need something. They will keep your loved one sitting in the corner for hours at a time, I mean HOURS! And don’t even bother pushing the call button. The Social Worker is absently awful. If you ask her questions, she gets defensive and will argue with you. She is supposed to be there to help not make you feel bad or make you more upset. The doctor in charge of this facility is not organized whatsoever, she has no idea what's going on there. Unfortunately, our experience here was ugly and sad. The social worker could have made it a lot better, but she made it 10 times worse.

He was having a stroke (has a history and they know its on file) and they didn't even notice it they just said "he wasn't acting like this yesterday" It was clear signs of a stroke, he wasnt speaking and couldnt hold the phone. Last words and the only word he said was "darling.." he ended up drowning from the fluid in his lungs causing him to be sent to Bethesda Hospital, died right in front of me the hour I got there.. DONT COME HERE LOOK ELSEWHERE I WISH I DID!!!
My uncle has been there just 3 days, and the people there are so compassionate and kind, I honestly can't thank them enough. They go out of their way to set up a meeting for you so you know everything that's going on with your loved one. This place showed me how my uncle should really be cared for. He was at a facility before, some odd months ago just briefly for 2 weeks if that, and the difference is just night and day. I recommend this place to anyone who has a love one in need of a rehabilitation center. The staff is so sweet, Dorrel & Roxanne are amazing (sorry if it's mispelled). Michelle who set everything up was just a breeze to talk to. I can't thank the staff enough for their patience and time.. Thank you Boulevard Rehabilitation Center!

[review] => My husband was admitted due to a fall in Oct 2021. One night aide, during the week, who was studying to be a nurse, was wonderful. The other one gave me a hard time many times when I requested something. The weeknight nurses were very cooperative.
My husband, who had dementia, often tried to get up from his wheelchair, forgetting that he could not walk by himself. One day, the last person to be with him was the PT person. She did not secure him properly next to the desk and he got up and fell. Off to the emergency room he went and came back with multiple bruises on his arms and knees. One day, I got a call to call his orthopedic doctor because he broke his thumb. When I came in, it was swollen and black and blue. They never wrapped it and the orthopedic doctor was very upset. I asked him what happened. He said it happened in the bathroom. I accompanied him with the aide many times, and the only way that could have happened, was if someone was not paying attention to him getting up and he tried to grab the rail and him it very hard. I asked the administrator to find out what happened. She never called me back. He also got pneumonia. I heard the coughing, told them and they said he was ok. Two days later, I got the call that he had it and was on antibiotics for 5 days. I was there every day during the month he was there from afternoon until he went to bed. The only reason I chose this place was that it was the only one at the time, that would allow me to spend the time with him and because he depended on me and that was important to be there. They do not have good ratings.

[review] => I had a family member there and they were horrible to her. She had a stroke which left her unable to use her right side! The night shift people were especially horrible. She needed to go to the bathroom and rang for ever till finally she could not hold it and wet herself in her bed waiting! She called out and no one came! She spent the whole night in a wet bed and hospital gown. Then next morning the “nurse” told her that she was dry and not to complain. They would put her in a chair and leave her there in a corner for four hours till it was food time. They lost her teeth (bridge) and her prescribed eye drops. She was fed mashed potatoes for breakfast and broth all the time because she had not teeth to eat with even though they lost them! The therapy was sparse and on one occasion a very young therapy guy reacted to her pain complaint by tossing her back in half way out of her bed and stoping her therapy and walking out . She almost fell out of bed trying to fix herself beck into bed.
There is no doctor on duty or available to see you if you need help only nurses who really do not care!
Stay away from this place!

When I questioned why she was receiving so little and why she was doing her arms she said she was an outside contractor. She was left in bed with a depends which they would change once a day, They do not shower the residents. I had to clean her hair using dry shampoo. Food was subpar and served barely warm, one day the forgot to feed her dinner and when I called the desk they gave her a cheese sandwich. Her dirty clothes were kept in a cardboard box on the floor and never washed. My sister was visiting one day and had to kill a roach on the floor. When she reported it they said they come from the hotel next door. When ever you ask questions the staff tell you there busy and you have to wait. She ended up leaving this place in worse shape then when she got there. Keep your relatives out of this place, they are clearly there to make a profit and do not provide the services that they state on there website.

[review] => This facility is shameful with the exception of Donna Lee. My 91 year old mother was sent there after a complete hip replacement and several complications. Whomever they contract with for transport wanted to pick her up from the hospital at midnight! Imagine after a serious surgery thinking that was okay? When she arrived the next day, nobody thought to contact the family to check in. Because of Covid they have many restrictions, which I appreciate, but when we tried to drop off some of her belongings we were told they had to be inventoried and laundered. She wouldn't get her things for 2-3 days. My mother sank into a deep depression (which nobody mentioned for days). Every time we called nobody answered the phone at the nurses' station or a very sour nursing supervisor did and was incredibly curt. My mother was there for four days and it took that long to get her discharged. Once we got through to someone they obliged. When we saw our mother, I cried.......she had not had a shower, nor any physical therapy which is why she was there. She is now getting PT at home, healthy food and lots of rest in her own bed. She is happy again and gaining strength. Don't go near this hell hole. When she was discharged the aide handed us about 10 pages of new prescriptions and some notes. Nobody even had the decency to go over any instructions with us. I could continue but I think you get the message! Steer clear of this disorganized chaotic place.

Turn off the nurses cell phones. Many of them, not all, pay more attention to their phones than their patients.
Just saying.
Not bad, but hardly great! Response times are not the best, sometimes as long as 90 minutes. So don't be in a hurry, or walk out to the nursing station.
As for the food, 2 stars. Mostly served at room temperature. One day my hot food was cold and my ice cream was melted. Go figure. The heating plate is generally at or only just above body temperature. They do have some good food, and well prepared, it's unfortunate that it can't be served at the right temperature.
The staff are kind and caring for the most part. Friendly and cheerful. 5 stars there. Most are skilled and capable, RN, LPN, CNA's for the most part. Occasionally you may find someone not experienced in a particular aspect of care, but they can't teach everything in school.
Overall, a decent place to go to recover. Oh, yeah, don't forget to meet your guardian angel. They can be a great help!

The aids have very thick accents, which would be fine if they would speak up! They have lousy attitudes and mumble answers (if they answer at all). This does not work when most of the patients are elderly and hard of hearing to begin with. They are terribly unresponsive. We rang for assistance for my mom one evening and waited 25 minutes before I walked to the desk and found 3 employees just sitting around chatting and ignoring all of the call lights in the wing. The nurses try their best but are severely understaffed.
My mother entered the facility to rehab from a broken hip and left with a perforated colon which led to major surgery and her death 2 weeks later due to the incompetence of the doctor on staff.
We were warned by hospital staff to choose a different facility but my mom insisted on Boulevard because it was close to her home.
To add insult to the injury done to my mother, while we were planning my mother’s funeral after sitting with her in the hospital and hospice for several weeks, we received a threatening letter saying that they were sending our bill of $120 to a collection agency! DO NOT SEND YOUR LOVED ONES TO THIS FACILITY!

[review] => I visited a family member who is a patient at Boulevard Rehab. I brought lunch since she told me most of the food was poor quality and tasted awful. Also the kitchen staff did not pay attention to her dislikes and continued to serve them although they were clearly written on her meal ticket. I arrived at noon and was upset to see that she was just getting dressed and out of bed. Upon questioning the staff I was told each CNA had 10 patients wash and get ready. I asked my relative about the service and she said that after pressing the call light for help she usually has to wait from thirty to forty-five minutes for a CNA. Most of the residents are kept in diapers all day and night and are not changed often enough. Also showers are not given frequently. Her clothing sent to the laundry is lost, but they recently started to put name labels on items, so that may improve. Sometimes certain medications are missing and patient is told that they are on order at the pharmacy. The diaper supply was empty for days and I had to buy diapers for her. Patients with obvious dementia are allowed to go through the corridors all day screaming. It's obvious that they need proper organization and adequate staffing.

Went to the rehab and within 3 days I had to have her removed and transported back to the emergency room! Why...The staff there does not have the correct equiptment that she needed! Walking down the hallway all you see are people who really need help and are left there to die so they can make room for another patient! I have no idea what these so called nurses are there for! There are a few that are nice and professional but the rest are just there to make a fast buck! Sad but true! What happened to our health care system? Who is inspecting this facility? The last day I went to see my mom, (before I had her removed,) I saw an elderly man in a wheelchair in the hallway! It is clear that they are short staffed, and the CNAs were often too busy chit-chatting among themselves to even pop their head in for 10 seconds to see if the patient is ok. They also didn't help my mom turn every few hours, so her bedsore was worse going out than going in. I also saw other residents in wheelchairs in the hallways for HOURS without any water cups - I had to wheel one poor woman back to her room myself after she begged me.
The only reason I give Boulevard 2 stars instead of 1 is that a few of the nurses were really sweet, attentive people (but clearly stretched to their limits), and the rooms weren't bad.

I found her disheveled, unwashed, and hallucinating when I came to pick her up for a doctor's appointment. I signed her out against medical advice and immediately took her to her primary care doctor who said that the hallucinations were most likely from a urinary tract infection. She advised me to take my mother immediately to the emergency room which I did. Upon testing in the hospital, she was found to be dehydrated and to have a urinary tract infection for which she needed intravenous antibiotic. She was kept in the hospital for 3 days.
DO NOT BRING YOUR LOVED ONES TO THIS FACILITY!!

In addition they had lost her purse, checkbook, credit cards for a few days which caused numerous headaches, thank God they finally found them! They should be audited by the state again which gave them a miniscule one star rating, I believe they were too generous!

[review] => My grandmother was in the hospital, and my mom and I had to visit a number of facilties in Boynton Beach. We went to them all and toured each one.
We went to Bouelvard last, we should have gone there first. First of all during the tour the staff at Boulevard were extremly friendly, compassionate and caring they took the time to explain the therapy and treatmetns that grandmom would receive. They answered many questions and even encouraged us to talk with other families and patietns. TWe did. We met a number of patinent and their families; who were visiting, we talked with about six families, they all were extremely hjappy.
Three of them told us that they had actually transfered from two other nursing centers in town that we had also looked at. One had spent a million or so dollars on rennovations,had an internet cafe... but if your staff are miserable and unhappy, and the patients were all complaining aout the staff speaking in creole in the hall ways, and unaswered call lights..we witnesed an aide go in and shut off a light and leave... well....The other told us about their expereince at another large facitliy..the buildign was newer,,but bricks and glass and brass do not proivde care, and apprently neither did their staff..
So we went to Boulevard, and we have not been disappinted. The staff there get it. They are focused on the patients,, they all seem to get along and truly love their jobs. I found out that many of them have or had family members in there. I can understand why. The Center is extremly well run, it is clean, and cheerful. My mom does not worry about her mom being at Bouelvard, I could tell you that we would have in the other nursing centers. I want to say one more thing..if you are out of town like up north and your family member is here...do not go anyplace else, you can trust the staff at Boulevard to care for your loved one like they are their own family. Somehow they get this and live it. Great Job Bouelvard.

I want to start by thanking all the staff at Boulevard for the care and compassion each provided to my mother.
My mother was a long term resident at Boulevard for about two and a half years. After two hip replacement, two strokes, many falls and other medical issues, home health care was just not enough anymore. As much as my mother did not want to leave her home, my brother and I had no choice but to seek more care for my mother. She lived up the road from Boulevard and we knew of the place as her husband was a resident there fifteen years prior.
My mother was not an easy resident as her mind was sharp, but her health issues continued to take a major toll on her body and mobility. I have seen first hand the compassion and help that the staff provided to and for my mother. Each time I would visit my mother at all times of the week, the place was extremely clean and welcoming. My mothers room was always well kept and many times I would see the staff helping and caring for my mother. Paulette was the Unit Manager on the wing that my mother was staying and she always would provide my brother and I with updates and the status of my mothers condition. She returned phone calls and I have to add spoke with my brother and I with suggestions to help aide us with my mothers conditions.
I met many CNA's that cared for my mother and all knew her by name and I always seen them care for my mother with compassion. I had the opportunity to talk with the Administrator named Tim and the Director of Nursing, Tanisha. Every time I would visit they were always out and about and interacting with not only the families, but the residents as well. I have seen them stop and talk with residents like they were life long friends. They new their names and situations and it always made me smile to see the care and compassion they have provided to all. They truly care and it shows. It shows with most of the staff I saw every time I would visit. It is truly refreshing these days to witness a collective group of people care for our loved ones as we would.
My mother passed away two weeks ago at the age of 84. I wanted to take a moment to stop and say thank you with my heart to all the staff at Boulevard. To watch a loved relative health fail and life slipping away is one of the hardest things a person has to do in life no matter what your faith my be. Boulevard helped me get through these past couple of difficult years. Thank you Boulevard for your caring a compassion and for providing such care.

I am At Boulevard rehab because of a stroke I had I am lucky to meet people who treat like I family sorry I don’t know last names. CNA’s Edwin, Limage, Mona, Ketaly, Suze, Marie, Russell, Beatrice, Louise, Nurse Kelly, cna Rosemarie, Kelly, Nurse Ricky, Marylou, Joanne, Michael, Michelle, Amanda, Antonio, Rosemarie, Tanisha, Faleine, Joanne, and. Suze, Kately, Stephanie, BERTHA, Marilou, Sophie, Dr. Daryl Nurse Nancy , Nurse Lovely , Nurse Micheal, Nurse Margarette, therapist Marylou, Michael, Joanne,Gwen, Ingrid, Patricia, I don’t remember the name of everybody. But I am blessed because people treat me like their own I am grateful.
At Boulevard I have been blessed to meet Allison and Sophie the activities director. For me it would be hard to be pleasant to everyone, but they do a great job. I am grateful because whether I poop Mona changes me. And everyone helps everyone. I will always keep that idea because I do believe many hands makes the work lighter.
